Minimally Invasive Hip Replacement, Explained.
The Direct Superior Approach (DSA)
Mr. Makrides and his fellow Consultant Orthopaedic Surgeons are leaders in minimally invasive hip replacement surgery, particularly using the Direct Superior Approach (DSA). This advanced technique is an evolution of traditional hip replacement methods, designed to minimise tissue damage and accelerate recovery.
What is the Direct Superior Approach (DSA)?
The Direct Superior Approach is a muscle-sparing technique for hip replacement surgery. Unlike conventional posterior or lateral approaches, DSA allows access to the hip joint without cutting key stabilising muscles or tendons. Instead, the surgeon makes a small incision just behind the hip, carefully working around essential structures rather than through them.
Benefits of the Direct Superior Approach
Muscle Preservation. The DSA avoids cutting the gluteus maximus and external rotator muscles, which are crucial for hip stability. This leads to better long-term function and reduces the risk of post-operative limp.
Faster Recovery. By preserving key muscle groups, patients experience less post-operative pain, reduced swelling, and quicker return to mobility. Many patients can walk unaided within days of surgery.
Reduced Risk of Hip Dislocation. The DSA maintains natural hip stability, lowering the risk of dislocation compared to traditional posterior approaches. This allows for greater confidence in movement after surgery.
Improved Aesthetics. The incision used in DSA is typically shorter and more discreet, reducing visible scarring and promoting faster wound healing.
Less Post-Operative Pain & Reduced Blood Loss. With minimal soft tissue disruption, patients often require less pain medication and have lower intraoperative blood loss, reducing the need for transfusions.
Quicker Return to Normal Activities. Many patients regain mobility within weeks, returning to work, driving, and even engaging in sports sooner than with traditional hip replacement techniques.
Your Minimally-Invasive Hip Replacement Journey
Patient Co-ordinator
Your Co-ordinator will guide you through scheduling your procedure, discussing the cost, and reviewing your medical history in detail. They will connect you with the right specialist consultant to ensure your treatment plan is tailored to your unique needs. They will ensure you feel informed and confident as you begin your journey.
Pre-Treatment Assessment
You may attend the hospital for diagnostic assessment, or for pre-operative checks if required. If you have existing diagnostic scans, you may not require them to be repeated. Our clinical team will ensure you are warmly cared for, and well prepared.
Consultant Surgeon
For your convenience, you will meet your surgeon by video call to discuss your minimally-invasive treatment plan. They will answer your questions and provide you with a balance of risks, and likely outcomes. You will meet them in advance of your procedure.
Post-Treatment
Due to our advanced surgical technique, you will begin your post treatment aftercare plan, before you are discharged. Our team is on hand 24-7 to deliver the care you need, when you need it. Whilst we will provide you with walking and in-home aids, typically patients will walk unaided in the days following surgery.
Stages of Recovery
Our skilled medical team is committed to supporting you at every phase. Immediately following your procedure, if you are not discharged the same day, you can expect to stay in the hospital for 1 night, where you will receive attentive care and begin initial post-operative physiotherapy under supervision.
Once home, you may need assistance with daily activities to ensure a safe environment as you navigate around with your new hip. You will have scheduled calls with the hospital team to check on process. You may take short walks using a walker or crutches provided until comfortable enough to walk unaided.
You will be guided to use techniques such as ice & heat stimulation to help and post-surgery pain. Motion exercise and gait training will be guided which will help ensuring safe and effective walking techniques to promote proper joint mechanics, and prevent limping.
By the six-week mark, significant improvements should be noticeable. The physiotherapy regimen will then include more demanding exercises focused on endurance. Low-impact activities such as stationary biking or swimming are introduced to enhance cardiovascular health without stressing the new joint.
Rehabilitation doesn’t end at three months, Maintaining the function of your new joint is a lifelong endeavour. Ongoing conditioning includes low-impact exercises such as walking, cycling, or aqua aerobics. Check-ups to your physiotherapist are recommended to ensure optimal effectiveness of exercise routines.
